How to prepare for your appointment

1. Before your appointment

2. During your appointment

Upon arrival you can expect to be greeted at check-in before promptly being escorted back to an exam room. Remember to talk about your medical history, current health concerns and future health goals.

3. After your appointment

As a Medicare Advantage health plan member, you may receive a survey from your health plan that will ask you questions about your experience with your PCP. This short survey may be given over the phone as an automated message from your health plan or may come in the mail. If you do receive a survey and need assistance, we are here to help you in any way possible as this is an important survey to ensure that your medical care is the best.

60-90 days after your appointment

Seeing your PCP
Once you arrived for your appointment, did you wait long to see your PCP?
Getting needed care
